Find out about modifications by consent for SUP 16.12.5R, SUP 16.12.11R, SUP 16.12.15R, SUP 16.12.22AR and SUP 16.12.25AR (all 'Note 5').
This modification will allow firms that have a CAD2 waiver not to submit Form FSA006 where the information contained in Form FSA006 is already supplied as a condition of the firms' CAD2 waiver.
This modification removes the requirement for the firms to supply two sets of the same information in different formats.
What the modification means for firms
The practical effect of the modification is that a firm which has a CAD2 waiver and which supplies information in accordance with appendix 1 of that waiver will not be required to complete Form FSA006.

The modification is valid for as long as the firm has a CAD2 waiver.
